Emergencies
If you or one of your family members are having an emergency, call 911 or proceed to the nearest emergency room. If the need is less urgent, call us. If we can safely manage your problem with an office visit we will be happy to do so. If we feel you need an evaluation at a hospital we will expedite the process for you. We are affiliated with the hospitalist physicians at GMH and at the Charlotte hospitals.

At Each Office Visit
Each time you visit our office you will be asked to update your individual account information and have your current insurance copied. This information is vital in case we need to contact you regarding test results, or specialist appointments. We need copies of your insurance cards to file your insurance correctly and for authorizations and referrals.

Bring all your medications, or at least a careful list, with you to each office visit. This includes medications that have been prescribed by another physician or supplements you may take over the counter

Appointments
Regular office hours in both Belmont and Stanley are by appointment only. You may also request non-urgent appointments through our website. During all Urgent Care hours you may walk in to be seen on a first come first served basis. If your provider has asked you to return to the office for lab work or if you would like your blood pressure checked, those walk in hours are Monday through Friday from 8:00 AM till 4:30 PM. The lab is not open during Urgent Care Hours.

Cancellations
Please call 24 hours in advance to cancel your appointment. This will allow someone else who may be sick to be seen in your place.

Prescription Medication Refills
For routine medication refills, please contact your pharmacy. They will in turn contact us with the accurate refill information we need. You may also use our website to request refills. Certain medications, such as controlled substances, will not be filled on weekends, holidays, or evenings.

Telephone calls
Routine questions should be phoned to the office Monday through Friday between 8:00 AM and 5:00 PM, or you may use our website. A message will be taken for a doctor to review. The clinical staff will be able to answer your questions when they return your call.

Specialist Appointments, Referrals, and Authorizations
Read your insurance information carefully. Many plans require authorization paperwork to be completed by our office before seeing other specialists. Call the "800" number on your insurance card or our office if you have questions.

Medical Forms
If you have a form to be completed we as that all of your information is filled in before leaving the form with us. There may be a charge for certain forms to be completed. We require seven business days to complete medical forms.

Payment Policy
We participate with most insurance companies (see listing under the About Us tab), and we will file your insurance claims for you. Co-pays, co-insurance and deductibles are expected at the time of service. We will need a copy of your insurance card at each office visit to insure that we file your insurance correctly, and to correctly perform authorizations and referrals. If you do not have your insurance card with you we will need to reschedule your appointment.

There is an additional $35 fee for Urgent Care above the usual office charges. This will be filed with your insurance company, along with the usual office visit claim.. If your plan does not cover the after hours fee, you will be billed for this amount.
